1. Copy of a voided check or letter from bank stating the account holder’s name, routing number and account number.
   
   (NO STARTER CHECKS!)
2. Copy of current E&O
3. AML Training certificate

If AML was taken through LIMRA, please provide a screen-shot showing your name, AML course and completion date if possible. Since LIMRA does not provide an actual certificate, we can submit a screen-shot to help speed up the process of verifying AML.
